MHSAA Health Assessment (Physical) Form
All student-athletes (including Academy) are required to have a current MHSAA Physical Form on file with the Athletics Office. To be current, a physical form must be completed by a medical professional after April 15, 2025, for the 2025-2026 academic year.
All new incoming student-athletes must have a Health Appraisal Form on file with the Nurse's Office AND an MHSAA Physical Form with the Athletics Office. We suggest having a medical professional complete both forms at an annual physical.
