State of Michigan CA-6 and CA-7 Employment Forms
The State of Michigan requires any child under the age of 18 to fill out a "Combined Offer of Employment and Work Permit/Age Certificate". If your son intends to work at anytime during the school year or breaks, please click on the corresponding link below and follow these instructions:
- Download/print and parent/guardian fill out Section I of the form
- Take to the employers to fill out Section II
- Take to U of D Jesuit Student Affairs Office to fill out Section III
- Return to employer
Health and MHSAA Forms
The following healthârelated material is required by the Health Office for each student attending University of Detroit Jesuit High School and Academy. Please read through the information below, carefully, to ensure your student is compliant.

Please be honest and forthright regarding your son’s medical history, especially as it pertains to prescription medication. This information is an integral component of your son’s care in the event of an emergency.

If an allergy, condition, or medication is not seen as an option in the MHSAA Physical Form, please email the School Health office to complete the health history.

Release for Dispensing of Medication Forms
- An Authorization to Administer Medication form is required for ANY/EVERY prescription medication used by a student during the school day each school year. This form applies to those medications deemed self-carry (i.e. inhaler, insulin, Epi-Pen) or those administered in the clinic by the nurse during the day (i.e. Ritalin). This form must be signed by a physician and submitted through the Magnus Health platform.

New Student Requirements

An updated physical exam, health history and immunization records are required for all NEW students. All new students will use the MHSAA physical form to be completed by your health care professional, even if they do not intend to participate in athletics while at U of D Jesuit.

The MHSAA Physical Form can also be used to satisfy the MHSAA annual physical exam requirement for student-athletes.

This form DOES satisfy the requirement of the signed physical required for participation in high school or Academy athletics (MHSAA Sports Physical form). To be valid for the 2026-2027 school year, the annual physical exam and form need to be completed on or after April 15, 2026.

Current Student-Athlete Requirements

All student-athletes are required to submit an MHSAA physical Form completed by your health care professional after April 15 of the calendar year.

Please inform the Student Health Office about any major changes in a student’s health history by submitting a new physical form or by contacting the Student Health Office directly.
